If you take care of someone who is elderly, disabled, or has health issues, You need to be documenting every day as to what transpires. You don't have to call it a chart, you can just call it Moms Diary. But it is important. And will only become important when things start to go bad. If you have to go to ER and they ask: When did she have her last blood thinner? When was her last BM? How long ago did this symptom start? When did they change this dose of medication? When you go for an annual check up and you want to explain to the doctor that her condition now has deteriorated from a year ago, you have a record to show specifics to show the trend. You can show that Dads blood pressure has steadily increased over the last 6 months and today's high reading is not an isolated reading. When problems occur it is a lot easier to grab the BOOK rather than hunt for all these things and get to ER only to find that you forgot the medication list. The other factor is when you talk to your doctor either at his office or at ER, when they see how involved and organized you are, you are taken a lot more seriously than when you are standing there upset because of the emergency and your brain goes blank and you cannot remember details.
